Design an enquiry form with useful questions, clear labels, helpful error states and a reliable handoff to the person responding.

Every field in an enquiry form asks a visitor to do work. That work should help the next conversation. A well-designed form balances the information your team needs with what a prospective client can reasonably provide before speaking to you.
Give every question a purpose
List what your team needs to decide after receiving a request: whether it fits the service, who should respond and what preparation is necessary. Map each proposed field to one of those decisions. Remove questions whose answers nobody uses.
For a hypothetical website project form, a short description of the problem may be more useful than a required list of preferred technologies. A prospect can explain that publishing takes too long without knowing which integration or CMS architecture will solve it. Let people describe their situation in their own words.
Separate essential information from useful context
Require the information necessary to respond. Mark optional questions clearly. If budget or timing affects qualification, explain why you ask and provide a reasonable option for uncertainty. A person who is still scoping the project should not need to invent an answer to continue.
Use examples to clarify broad questions. “What needs to change?” can include a short hint about content, design or workflow. Avoid a paragraph of instructions inside a placeholder: it disappears when the visitor types and can make reviewing the answer harder.

Design the difficult states as carefully as the form
W3C’s forms guidance covers labels, instructions, validation and notifications. Treat these as part of the initial design. Show a persistent label for each control and explain errors in language that helps the visitor correct the problem.
Test an empty submission, an invalid email address and a failed connection. Check whether previously entered text remains available. On mobile, confirm that the keyboard suits the field and that error messages do not appear outside the visible area without any indication. The visitor should know what happened and what to do next.
Follow the request into the team’s workflow
Send a test request through the actual destination. Confirm the recipient, field values and any routing rules. The visible success message should reflect what the system has actually completed. A successful submission does not necessarily mean a meeting has been booked.
Name a person responsible for monitoring failed deliveries or unassigned requests. Keep a simple test record with the date and destination. After launch, review whether the information collected helps the response. If the team repeatedly asks the same follow-up question, decide whether it belongs on the form or in a later conversation.
